It contains multiple assistance programs designed and managed around the specific needs of each individual, upholding a non-judgmental approach and unconditional positive regard built on a sense of safety. We also provide services to assist participants in transition from the hospital or rehabilitation facility, focusing on moving towards living in an independent environment

Assistance - Personal Care Activities
Individuals with special needs often require support for everyday tasks, including personal hygiene, household cleaning, meal preparation and cooking, and mobility assistance. Our team was selected based on how they uphold our organisation’s values and mission with sound qualifications and experience in their field, which matched their desire to offer care and empathy to those most vulnerable in our community. We will be there with you on each step of this journey, providing exceptional care and support to you and your loved ones

Assistance – Travel/Transport
CCNA provides a range of supports related to transportation. Individuals living with a disability often struggle to stay mobile, which can have a significant impact on daily life. Our trained and experienced support staff will assist in accompanying the participants to their appointments, visiting their family or friends, or visiting their favourite café. Assistance is provided to accompany participants on taxi rides, in community vehicles, or the support staff’s car. In many situations, a support worker with the appropriate experience, driver’s license, and relevant insurance coverage can accompany the participant to provide ongoing care during their outing.

Community Nursing Care
At Compassionate Care Network Australia, we focus on a person-centred holistic approach. Our community nurses are compassionate and experienced with relevant screen checks. We endeavour to treat all our participants with professionalism and quality care that is met with a group of unmatched commitment. Our nurses focus on tailored support to always respect the wishes of participants. Our team of nurses is AHPRA registered, clinically experienced, and 100% dedicated to individual participants’ needs. We work with integrity and patience to satisfy the needs of those we support.

- In-home visits and provide an opportunity to collect feedback from participants on how our service can improve
- Prompt medication

Daily Tasks/Shared Living
We take exceptional care in choosing our staff who represent our Organisation to ensure they embody all aspects of integrity in everyday interactions. You can rest assured that our trustworthy team will treat your home as theirs and be honest in all interactions. Our support staff have at least six months’ work experience and are competent

Development – Life Skills
Compassionate Care Network Australia employs qualified and compassionate Support and client Coordinators who are dedicated to matching you with our experienced support staff, who can work with you to ensure your needs are met while supporting your plan goals.

Household Tasks
People living with special needs often struggle to complete household tasks. This can have a severe effect on life quality and confidence, along with personal safety. Depending on the needs of each individual, our experienced support staff can assist with various household tasks. Whatever services we provide, it is with kindness, compassion, and respect.

Therapeutic Supports
Our organisation focuses on a person-centred holistic approach. Our professional and compassionate team includes recognised therapists, individual and group counsellors. Our Counsellors are registered with the Australian Counselling Association (ACA) or other relevant bodies. We can assist participants with specific modalities. Our compassionate team is committed to supporting each individual in their journey as they access community engagement programs with the focus to achieving their goals. We respect each participant’s choices and commit to walking alongside them throughout their journey. We provide counselling support to those who may be experiencing the following:
- Pre-Grief support
- Grief and loss
- Anxiety
- Manage the day-to-day stress.
- Anxiety
Support Coordination (Level 1 & 2)
Our organisation takes a holistic approach when working with our participants. Our professional and compassionate Support Coordinators devise support plans that identify and sensitively respond to the participant’s culture, diversity, values, and beliefs. Participants are supported regardless of sex, race, marital status, sexuality, background, or cultural beliefs. Our assessment process identifies and incorporates the participant’s culture, diversity, values, and ideas within their support plan. Consultation with participants allows the support plans to deliver appropriate services provided by the staff of choice, including culture, beliefs and values when providing support. The collaborative process ensures relevant services are offered. Referrals are made if necessary. Support plans detail how participants’ designated supports are practised and their culture maintained.
